BREAST PROGRAM NURSE NAVIGATOR, NEW ENGLAND CANCER SPECIALISTS

New England Cancer Specialists (NECS) is currently seeking a Full-Time 32-hour Breast Program Nurse Navigator. The Breast Program Nurse Navigator will coordinate care for all NECS breast referrals for benign, high-risk, and malignant patients. This nurse will coordinate care for all breast patients referred to NECS for surgical, medical and radiation oncology care. The successful candidate will help navigate the patient through diagnostic workup to surgical and medical interventions and/or other clinical care needed to ensure the best outcomes for the patient such as imaging, medical oncology, physical therapy, genetic testing, radiation oncology and other supportive services. The ideal candidate must have excellent communication and organizational skills and a strong commitment to providing high quality patient care. The successful candidate must have two years of experience in surgical specialty or related field such as breast oncology.

Essential Duties and Responsibilities:

  • This individual will work closely with NECS intake department and triage benign, high-risk, and breast cancer referrals to the appropriate department.
  • Collaborates in developing and monitoring a measurement system that ensures services perform within defined standards.
  • Provides holistic care, education, support and advocacy to the patient and family.
  • Identifies and develops resources to meet breast patients' educational and psychosocial needs.
  • Works collaboratively with the breast care team.
  • Demonstrates the ability to articulate the breast services in a positive manner to health care providers and the public.
  • Serves as a consistent contact for patient/family throughout diagnosis and treatment.
  • Coordinates appointments for consultations and physician visits.
  • Maintains two-way communication with physician offices to improve the process of coordinating care for breast center patients.
  • This individual will provide care to our surgical patients throughout their entire experience, from pre-surgery to post-surgery to medical oncology and beyond.
  • Works closely with the oncology team to develop and implement individualized care plans.
  • Works well with other NECS navigators and other departments such as Radiology, Pathology, and supportive services.
  • Organizes and leads case conferences, tumor boards, and patient care meetings

Knowledge/Skills/Abilities:        

  • Knowledge in breast and/or surgical care with motivation to acquire additional knowledge in these areas. Proven organizational, communication and interpersonal skills.
  • Proactive initiative to identify issues and recommend solutions. Ability to handle multiple priorities.

Education and/or Experience:                                            

  • Bachelor’s degree in nursing required.  Licensed Nurse within State of Maine required.
  • Two years plus nursing professional experience required.
  • Surgical/Breast Oncology experience preferred.
  • OCN required within 1 (one) year of hire or when eligible based on ONCC criteria



HIGH RISK & SURVIVORSHIP NURSE NAVIGATOR AND WELLNESS HEALTH COACH, NEW ENGLAND CANCER SPECIALISTS


The High Risk & Survivorship Nurse Navigator and Wellness Health Coach at New England Cancer Specialists is responsible for providing personalized health coaching and supportive care coordination for cancer survivors and high-risk patients. This role plays a key part in patient education, follow-up, and engagement, ensuring a holistic and informed approach to long-term wellness. This position is an essential role in the Survivorship and High-Risk Cancer Genetics Clinic.

Duties/Responsibilities

Provide one-on-one health coaching sessions tailored for survivorship and high-risk patients.

  • Work closely with the Program Manager to serve as the Wellness Program Coordinatorwhich includes:
    • Educating patients about NECS wellness programming.
    • Enrolling eligible participants when appropriate
    • Conducting pre- and post-assessments on site with support of Clinical Medical Assistant.
    • Providing on-site support during wellness programs.
  • Work with NECS genetic counselors and High-Risk providers to ensure that high-risk patients have appropriate follow-up care.
  • Assess and triage the urgency of referrals for Survivorship and High Risk. Contact referrals with an introduction to the services provided and schedule in a clinically appropriate time frame.
  • Assess and triage clinical needs via DASH and CareSpace. Collaborate with Clinical Medical Assistant to meet patient needs in a timely fashion.

Education/Experience

Bachelor’s degree in nursing required. Licensed Nurse within State of Maine and New Hampshire required.

  • Certification in health coaching or relevant continuing education is preferred or to be obtained within 1 year of hire.
  • Two years plus nursing professional experience required.

DIVISION OF HEMATOLOGY/ONCOLOGY, DEPARTMENT OF MEDICINE

The Division of Hematology/Oncology at the University of Vermont (UVM) and the UVM

Medical Center, are seeking a medical oncologist/hematologist faculty member on the Clinical

Scholar Pathway at the Assistant/Associate level to join the Hematology/Oncology Network

division with a primary practice site in Plattsburgh, NY. Clinical duties will encompass general

oncology and hematology (benign and malignant disease) and include inpatient consult service

and outpatient clinics at University of Vermont Health Network Champlain Valley Physicians’

Hospital (CVPH) in Plattsburgh, New York, as well as at affiliated clinics in Elizabethtown and

Malone, NY. Weekday and weekend call on a rotation with other physicians. There will be

ample opportunities to interact with the University of Vermont Cancer Center for patient care and

participation in clinical trials. In addition, it is anticipated the faculty member will practice two

days per month at the UVM Cancer Center outpatient clinic in Burlington, Vermont with an

opportunity to focus on academic, subspecialty hematology/oncology.
